Clinical observation of 56 cases of abdominal trauma in war zone



Abstract
The report consist of 56 cases of abdominal injuries treated at the 00 hospital on battle area in
Viet Nam, with brief of literature for age distribution, incidence of injured viscera, lag time, number
of injured organ, associated injuries, agent of injury, blood group, postoperative complication, analysis
of death and mortality rate.
The results for cilnical observations were introduced as follows:
1. In age distribution, of the 56 cases, 35.7 per cent in the 23-24 years old age group, next
highest was the 25-26 old age group with 25.0 per cent. The third group was 21-22 years of age.
2. The injured organs were 30 cases(53.5%) of small bowel, 24(42.8%) of large bowel, 9(16.0
%) of liver, 7(12.6%) of stomach and kidney, 6 (10.7%) of spleen and urinary bladder and 1
(1.7%) of pancreas and gall bladder.
3. In the number of injured organs, 13 patients (23.2%) were single organ, 9(16.0%) were
two organs, 15(26.7%) were three organs and 19(33.9%) werre four organs or more.
4. In the lag time, less than 30 minutes were 6 cases (10.7%), one hour was 17 (30.0%), two
hours were 15(26.7%), three hours were 3 (5.3%), 2-6 hours were 6(10.7%), 6-12 hours
were 2 (3.5%) and over 12 hours were 6(10.7%).
5. In the associated injuries, 26.7 per cent (15 cases) were fracture, the next highest injury was
extremities (19.6%).
6. In the agents of injuries, gunshot and shrapnel injuries were 25 cases (44.6%) respectively,
one stab (1.7%), two automobile (3.5%) and three others (5.3%).
7. In blood group, A group was 12 cases (21.4%), B group 15 (26.7%), O group 22 (39.2%)
and AB group was 7 cases (12.6%).
8. In post-operative complications, wound infection was 21.1 per cent(12 cases), the next highest
complication was wound dehiscence with 16.0 per cent (9 cases), fecal fistula 14.2% (8 cases),
intestinal obsstruction and diarrhea 8.9%(5 cases) respectively and delirium tremens 7.1% (4 cases).
9. Results in surgery, of 56 cases, 43 patients (76.7%) were excellent, 6(10.7%) poor and 7
died. The post-operative mortality rate was 12.6 per cent (7 cases), one patient was died due to
homorrhagic shock immediately, two patients by heart failure, other two patients by renal faiure
and last two patients by peritonitis with sepsis during three weeks after operation.
